MacNamara emphasized that there were likely numerous civil servants and potentially ministers who now acknowledge they had crossed the line, especially referring to the controversial birthday party for former Prime Minister Boris Johnson that resulted in fines for him and Rishi Sunak. As Deputy Cabinet Secretary from 2020 to 2021, MacNamara played a significant role in the government's Covid response. She also disclosed her attendance at a rule-breaking drinks party on June 18, 2020, for which she was fined by the police. MacNamara admitted that the incident "shouldn't have happened" and acknowledged the rightful offense of the public.

In her written evidence to the inquiry, MacNamara recounted a meeting with senior No.10 staff on March 13, 2020, where she expressed grave concerns. She warned, "I think we are absolutely f*****" and predicted that thousands of lives would be lost. MacNamara further criticized the "toxic culture" within No.10, describing instances of individuals being subjected to "violent and misogynistic language." The inquiry also unveiled a text message where Dominic Cummings, former chief of staff to Boris Johnson, referred to MacNamara in a derogatory manner.
